Questionnaire Tip Basement Changed 9 Times & Reported Only 6

I applied to get my PR card renewed by myself. I had plans of travelling to India but today I checked my mailbox and found this big questionnaire. I have changed my basement apartment 9 times in the past 5 years but I have reported only 6 times to Service Ontario for updating my address on my driver’s license. For filing tax to CRA, I have only reported three addresses. How should I fill out my questionnaire? Can you please help me?

You have complicated the information to the provincial and federal governments by not providing the information on time. I would suggest you seek an appointment at my office or with any other professional who can sort it out for you. if you will do it by yourself, you are going to deteriorate more.
