    My cousin got divorced from her husband and she does not want to marry anymore. Her maiden surname was ‘Gill’ and she took ‘Sandhu’ which was her husband’s surname. Now she is divorced and want to revert to her maiden surname. What is the process? Can she start simply using ‘Gill’ again or she will have to go for formal name change?

    Could you please tell me about the previous name change done by your cousin? Was it a legal name change or she started using her husband’s name without any formal name change process?

      Yes. She changed her name earlier from Thunder Bay office in Ontario.

        She has to go through name change process at the same office again. She needs to apply for last name change from Sandhu to Gill and she will receive a name change certificate. She can use that certificate to change her last name on all other identification dcouments.

    She can apply for a name change in Ontario due to marriage, divorce, death of a spouse or any other valid reason. You have to be living in Ontario for at least 12 months. You need a name change certificate for any legal document changes such as PR Card, Citizenship Certificate, Passport, CPP record etc. It is always good to have your Legal name change certificate to avoid any problems.
