Use of Representative v. Authority Release Personal Info Forms

I am sponsoring my common-law partner and I have a question about the following 2 immigration forms:
– Use of Representative (IMM 5476)
– Authority to Release Personal Information to a Designated Individual (IMM 5475)
I read on the IRCC website that my partner (applicant) needs to sign IMM 5476 so that I can receive correspondence on his behalf? I am just confused because a friend of mine was telling me that I need to sign both of the forms (IMM 5476 and IMM 5475). Please help me with this confusion and tell me which form I need to submit along with my application.

If you have a representative (Immigration Consultant / Lawyer / Notary of Quebec), you will sign IMM 5476 form. If you are un-represented and want your information can be shared with your family member who is designated by signing a form, you will sign IMM 5475 form.
